4. Questions to ask when a referral is sent

  • Which clinic or specialist received the referral?
  • When was the referral sent?
  • Is there a reference or confirmation number?
  • Who should be contacted if no appointment is received?
  • Are additional tests or documents required?
  • Who is responsible for communicating results?

5. What to organize if no update is received

Record the referral date, destination, and contact instructions. Use the identified official clinic contact pathway for follow-up.
