When your clinic provides an Ocean form

What is an Ocean Form?

Ocean eForms are digital questionnaires that healthcare organizations can configure for patients to complete online or on a clinic tablet. A clinic may use them for administrative information, questionnaires, intake or follow-up workflows.

Ocean is a separate third-party platform. Only use an Ocean link supplied or confirmed by your clinic, and follow the clinic's instructions about what to submit.

Step 1

Your clinic provides a secure Ocean link

The link may arrive through an approved clinic website, message or other official channel.

Step 2

You complete the requested form

Answer only the questions requested by the clinic and confirm you are using the intended form.

Step 3

The clinic receives it in its configured workflow

Depending on setup, staff may review it in Ocean and import or synchronize information with the clinic's EMR.

Designed for secure exchange

Ocean documents that Cloud Connect uses a shared encryption key to encrypt and decrypt health information and SSL for secure transmission between Ocean and an integrated EMR.

EMR workflow depends on configuration

Ocean documents Website Form import support for TELUS PS Suite, TELUS Med Access, OSCAR, OSCAR Pro and QHR Accuro. The exact review and import process is determined by the clinic.

ProfileMD.ca does not collect the answers. The secure form and its data handling are operated through Ocean and the healthcare organization.
